State Approaches to Consumer Assistance Training
With open enrollment underway, exchanges are mobilizing their efforts to provide assistance to individuals who will begin enrolling in coverage options in October. This chart highlights the approaches states have taken to develop education and training for the individuals who will provide enrollment assistance through Navigator, In-Person Assister (IPA) and Certified Application Counselor (CAC) programs.

[3] According to final regulations, federal navigator training will include training on QHPs, insurance affordability programs, tax implications of enrollment decisions, basic concepts about health insurance, eligibility and enrollment rules, providing culturally and linguistically appropriate services, ensuring accessibility, understanding differences among health plans, privacy and security, working with individuals with limited English proficiency, customer service standards, outreach and education strategies, and administrative rules.
[4] Iowa is a plan management partnership state and is not taking on any consumer assistance functions.
[5] Due to federal funding rules, some SBEs are delaying the implementation of their Navigator program and are opting to have their IPA program provide the primary consumer assistance functions of the exchange during the initial stages of implementation.

[7] In September Texas Gov. Perry directed the Texas Department of Insurance to develop additional regulations for federal navigators, such as requiring navigators to complete at least 40 hours of state training in addition to the mandatory federal training, submit to regular background and regulatory checks and other requirements.
[8] In UT the federal government will operate the individual exchange and the state will operate the SHOP exchange as well as a SHOP-specific Navigator program.

For individuals living with complex, often chronic conditions, and their families, palliative care can provide relief from symptoms, improve satisfaction and outcomes, and help address critical mental and spiritual needs during difficult times. Now more than ever, there is growing recognition of the importance of palliative care services for individuals with serious illness, such as advance care planning, pain and symptom management, care coordination, and team-based, multi-disciplinary support. These services can help patients and families cope with the symptoms and stressors of disease, better anticipate and avoid crises, and reduce unnecessary and/or unwanted care. While this model is grounded in evidence that demonstrates improved quality of life, better outcomes, and reduced cost for patients, only a fraction of individuals who could benefit from palliative care receive it.
