Kim Yeagle

Missouri Coalition for Community Behavioral Healthcare
Kim Yeagle is a Licensed Clinical Social Worker since 1994. She completed her Masters of Social Work from the University of Missouri in 1991. She has worked in many areas of social services and behavioral health such as inpatient psychiatric, out-patient therapy with youth and adults as well as adolescent offenders, hospital social work and integrated health. Most recently she was the Healthcare Home Director for a Community Mental Health Center and led that agency in becoming a health home. Kim was the co-recipient of the first Healthcare Home Director Excellence Award in 2013. Kim joined the Coalition for Community Behavioral Healthcare in November 2014 and has provided clinical assistance in the development of Healthcare Homes and integrated care in the community behavioral health setting in several areas including, program policy, training initiatives, evidence-based practices, population health management, and clinical measures and outcomes.
Cross-Agency Collaboration: Integrating Pediatric Mental and Physical Health

For individuals living with complex, often chronic conditions, and their families, palliative care can provide relief from symptoms, improve satisfaction and outcomes, and help address critical mental and spiritual needs during difficult times. Now more than ever, there is growing recognition of the importance of palliative care services for individuals with serious illness, such as advance care planning, pain and symptom management, care coordination, and team-based, multi-disciplinary support. These services can help patients and families cope with the symptoms and stressors of disease, better anticipate and avoid crises, and reduce unnecessary and/or unwanted care. While this model is grounded in evidence that demonstrates improved quality of life, better outcomes, and reduced cost for patients, only a fraction of individuals who could benefit from palliative care receive it.
